Instant Drug Tests vs. Lab Tests: Which Should You Use?

Most drug testing programs use two kinds of tests: fast, low-cost instant tests to screen, and laboratory tests to confirm. Knowing what each one does helps you build a program that is both affordable and fair.

Side-by-side comparison

Instant (rapid) test Laboratory test
How it works Immunoassay strips in a cup or dip strip Immunoassay screen, then GC/MS or LC-MS/MS confirmation
Time to result About 5 minutes Usually 1 to 3 business days
Cost per test Low, especially in bulk Much higher
Result type Preliminary positive or negative Confirmed result naming the exact substance
Equipment None Certified laboratory
Best for Frequent, on-site screening Confirming positives, legal and federally required tests

How accurate are instant tests?

Quality instant tests are highly accurate at detecting whether a substance is above the cutoff level. Their limitation is specificity: an immunoassay can occasionally react to a related compound, such as certain medications, and it cannot tell you exactly which substance caused the result. That is why a preliminary positive should always be confirmed by a lab before action is taken.

When to use an instant test

When you need a lab test

  • Confirming a preliminary positive before an employment, housing or treatment decision
  • Federally mandated DOT testing, which must be processed by a certified lab
  • Detecting substances instant tests miss, or identifying the exact medication
  • Legal and court-ordered testing that requires chain of custody

The most cost-effective approach

Screen everyone with instant tests, then send only the preliminary positives (and any disputed results) to a lab. Because most results are negative, this keeps lab costs to a small fraction of what testing every sample at a lab would cost, while still giving every positive a confirmed answer.

Frequently asked questions

Are instant drug tests as accurate as lab tests?

They are accurate for screening, but they give preliminary results. Lab tests confirm the exact substance and are the standard for final decisions.

How long do lab drug test results take?

Usually 1 to 3 business days, longer if a sample needs confirmation testing.

Can instant drug tests be used for DOT testing?

No. DOT-required tests must be processed by a certified laboratory.

Should every positive be sent to a lab?

Yes, before any serious action is taken, especially if the person disputes the result or has a relevant prescription.
